When prospective residents are referred to the home it can come through different areas of referral. Sometimes it is where people literally drop in of the street.
Most often people are referred from other health professionals. These can be discharge co ordinators, social workers, GP`s, community mental health team or District nurses.
Wherever they come from we always are open to inviting the prospective resident and /or their relatives to view the home in person.
Once we have received a referral for someone and have a placement available Ella (Registered Manager) or Kevin (Team Leader) will contact the individuals responsible for their care and undertake a pre admission assessment to ensure we can meet the needs of the individual. This involves gathering all medical and health needs about that person and also personal history and what their hopes and aspirations are for their future.

Once the pre admission assessment is complete and both parties are happy to proceed the individual will be admitted to the home for a trial period of 4 weeks. This is applicable to both parties the resident, and the Home.
A 4 week review takes place and as long as both parties are happy the individual will become a permanent resident at The Sands.
Once permanency has been established the finance team will set up necessary details forpayments.
If the individual is not with Ashtree’s. We register them with Ashtree’s. This ensures they have regular reviews by the GP and able to access their repeat medication, dressings ect.
Any ongoing hospital / clinic appointments are maintained just as they would if the individual still lived at home.
Chiropody and hairdressing can be provided within The Sands Meadows.
